財團法人台灣網路資訊中心蒐集個資聲明
財團法人台灣網路資訊中心（下稱「TWNIC」）對於使用者透過域名濫用通報/申訴窗口所填寫的個人資料，包括：姓名、單位名稱、電子郵件、證明文件、及相關問題回覆等（依使用者實際填寫的個人資料而定），將僅於回覆通報/申訴需求之目的範圍內進行處理及利用。
TWNIC將於台灣地區及上述目的存續期間內，以人工、自動化機器或其他合乎當時科技之適當方式利用該等個人資料，除非回覆通報/申訴需求有必要或法律另有規定，TWNIC不會向第三人揭露該等個人資料。
依個人資料保護法第3條規定，就您的個人資料，您享有查詢或請求閱覽、請求製給複製本、請求補充或更正、請求停止蒐集、處理或利用、請求刪除之權利，您得以書面郵寄方式（郵寄地址為臺北市松山區八德路四段123號3樓）行使上開權利，TWNIC將於收悉您的請求後儘速處理。
您得自由選擇是否提供相關個人資料，惟若您拒絕提供的個人資料，為TWNIC回覆通報/申訴需求所必要之資訊，TWNIC將無法辦理相關事項。其餘有關法定應告知事項，請參閱「財團法人台灣網路資訊中心隱私權保護政策」。
Taiwan Network Information Center Personal Data Collection Statement
The Taiwan Network Information Center (hereinafter referred to as the "TWNIC") will only process and use the Personal Data, including names, company/organization names, emails, identity documents, and answers to related questions (subject to the actual personal Data completed by users) completed by users over the domain abuse report/appeal within the scope and for the purpose of addressing the request for domain abuse report/appeal.
TWNIC will use such Personal Data by non-automated/automated means or any means appropriate to the contemporary technology within the territory of Taiwan and during the existence of the said purposes. Unless for the need to address the request for domain abuse report/appeal or as required by law, TWNIC will not disclose such Personal Data to any third party.
In accordance with Article 3 of the Personal Data Protection Act, you enjoy the following rights over Personal Data: To inquire, review, request for a copy of, supplement or correct, demand the cessation of the collection, processing or use, and erase Personal Data. You may exercise the above rights by post (mailing address: 3F, No. 123, Section 4, Bade Road, Songshan District, Taipei City). TWNIC will process your request as quickly as possible upon receipt.
It is your free will to provide Personal Data. When you refuse to provide Personal Data required by TWNIC to address your request for domain abuse report/appeal, TWNIC will be unable to address your request.
